Description: Cancer Care Plan Builder is a non-fee based technology that helps users create custom personalized Treatment Plans, Treatment Summaries, and Survivorship Care Plans for cancer patients, their primary care physician, and other members of their care teams. Some features of this technology include:
  • Forms for preparing treatment summaries
  • Surveillance guidelines and a schedule for follow-up care
  • Utilities like a regimen library, medical calculators and pick-and-choose forms Information on symptoms to watch for, possible effects of treatments and support resource
  • A library of articles and factsheets on survivorship care--any of which can be attached to individual Care Plans
  • A form for assessing and addressing the psycho-social needs of Survivors
  • Ability to create custom templates
  • Ability to customize Survivorship Care Plans with a practice logo
Data collected by this software is stored on a user`s local drive.

Technology/Standard Usage Requirements: Users must ensure their use of this technology/standard is consistent with VA policies and standards, including, but not limited to, VA Handbooks 6102 and 6500; VA Directives 6004, 6513, and 6517; and National Institute of Standards and Technology (NIST) standards, including Federal Information Processing Standards (FIPS). Users must ensure sensitive data is properly protected in compliance with all VA regulations. Prior to use of this technology, users should check with their supervisor, Information Security Officer (ISO), Facility Chief Information Officer (CIO), or local Office of Information and Technology (OI&T) representative to ensure that all actions are consistent with current VA policies and procedures prior to implementation.
