Description: SupraCCHT is software used for reformatting Care Coordination Home Tele-Health (CCHT) data. In a typical setup, the patient performs monitoring at home (Glucose, Blood Pressure, etc). This data is then transmitted to a secure website where it is available to CCHT clinical staff. The CCHT staff review the data, reformats and copies it to the patient`s electronic health record, and forwards it for provider`s review. The data presented on the website, when copied to an electronic health record (EHR) like Veteran`s Health Administration`s Computerized Patient Record System (CPRS), is not initially formatted for clinician`s use. This technology parses and reformats the data input from a CCHT program into actionable information.

SupraCCHT does all the processing in memory and does not store any data in any form. SupraCCHT does not make any connection with Computerized Patient Record System (CPRS) or Veterans Health Information Systems and Technology Architecture (VistA) databases, and does not read from, write to, or otherwise directly enter any data into CPRS or VistA databases.

Note: The latest version of SupraCCHT ( supports all three CCHT vendors: Health Buddy, VitelNet, and Viterion.
Technology/Standard Usage Requirements: Users must ensure their use of this technology/standard is consistent with VA policies and standards, including, but not limited to, VA Handbooks 6102 and 6500; VA Directives 6004, 6513, and 6517; and National Institute of Standards and Technology (NIST) standards, including Federal Information Processing Standards (FIPS). Users must ensure sensitive data is properly protected in compliance with all VA regulations. Prior to use of this technology, users should check with their supervisor, Information Security Officer (ISO), Facility Chief Information Officer (CIO), or local Office of Information and Technology (OI&T) representative to ensure that all actions are consistent with current VA policies and procedures prior to implementation.
