Description: The Benjamin Rose Institute on Aging (BRI) Care Consultation Information System (CCIS) is an evidence-based, personalized care-coaching program. This technology is designed to help professionals deliver cost-effective assistance and support to individuals with chronic conditions (such as Alzheimer`s disease, diabetes, and heart disease) and their caregivers by telephone and email. Through a series of phone calls and emails, Care Consultants collaborate with families to identify needs and create an Action Plan with steps to alleviate their concerns. Care Consultants follow-up on Action Steps with families and work with them to add to, or adjust, the Action Plan as their problems resolve, or change.

The BRI CCIS is a web-based application that operates within VA`s own server infrastructure (hosted on VA premises). All data is stored in a Microsoft Structured Query Language (SQL) Server database. Information stored includes: Name, Address, Date of Birth (DOB), Demographics (race, religion, ethnicity, gender, education, marital status, employment), Health Conditions, Assessment data, and Action Steps. Data is transmitted over a network connection using Transmission Control Protocol (TCP)/Internet Protocol (IP) to the SQL database. Data is encrypted both in transit and at rest.
Technology/Standard Usage Requirements: Users must ensure their use of this technology/standard is consistent with VA policies and standards, including, but not limited to, VA Handbooks 6102 and 6500; VA Directives 6004, 6513, and 6517; and National Institute of Standards and Technology (NIST) standards, including Federal Information Processing Standards (FIPS). Users must ensure sensitive data is properly protected in compliance with all VA regulations. Prior to use of this technology, users should check with their supervisor, Information Security Officer (ISO), Facility Chief Information Officer (CIO), or local Office of Information and Technology (OI&T) representative to ensure that all actions are consistent with current VA policies and procedures prior to implementation.
